You might be a bit apprehensive if your dentist recommends a root canal treatment, especially if it's your first. That's understandable: Popular culture has portrayed the root canal as an unpleasant experience.
But as a routine dental procedure, root canal treatments have been responsible for saving millions of decayed teeth. And, with local anesthesia, the procedure is painless. In fact, a root canal treatment stops pain that often results from advanced tooth decay.
So, let's take the mystery out of the root canal. Here's the 411 on this vital but often misunderstood dental procedure.
Why the name “root canal”? The terms for the procedure—root canal therapy, root canal treatment or simply “root canal”—arise from one of the principal parts of the tooth involved, root canals. These are tiny passageways that lead from the tooth's innermost layer, the pulp, to the tooth roots. While treatment often focuses on decay or diseased tissue within the pulp, the root canals can be infected too and must be included in the later filling process.
Who can perform a root canal? All general dentists are trained in basic root canal procedures. Depending on your tooth's condition, your family dentist may be able to perform it. But if your tooth has an intricate root canal network or some other complication, you may need an endodontist, a specialist in interior tooth and root treatments. Endodontists can perform advanced root canal techniques and have the specialized equipment to handle intricate cases.
What happens during a root canal? Although details may vary depending on the type of tooth and extent of decay, there's a basic process for all root canal procedures. After numbing the tooth and surrounding tissues, the dentist drills into the tooth to access the inner pulp chamber and root canals, then removes the diseased tissue and disinfects the empty chamber and canals. After preparing the canals, the dentist then fills the empty spaces. This, and subsequent sealing and crowning, protects the tooth from future decay.
After the procedure you may have some minor soreness for a few days, which is usually manageable with mild pain relievers like ibuprofen or acetaminophen. This discomfort will diminish with time, and your tooth will have a new lease on life.

Bad news at your last dental visit: You have a decayed tooth. And not just in the enamel—the decay has invaded the tooth's inner pulp and the resulting infection is threatening the supporting bone structure.
You're thinking that tooth is toast. Then comes the good news: your dentist believes the tooth can be rescued with a root canal treatment.
But then you begin thinking about how often Uncle Sid says he'd rather undergo a colonoscopy than have a root canal. Is the procedure really as painful and uncomfortable as popular culture says it is? What is a root canal really like?
First step: Things go numb. Uncle Sid is wrong: A root canal treatment is painless because your dentist will first make sure the entire area involving the tooth is anesthetized. This does involve injecting the local anesthetic deep within the tissues, but you won't even feel the needle prick thanks to topical anesthesia applied to the surface gums.
Second step: Drilling deep. After applying a protective dam to isolate the infected tooth from its neighbors, your dentist will drill a small access hole through the enamel and dentin to reach the pulp and root canals. If it's one of the larger back teeth, the access hole is usually drilled in the tooth's biting surface; in a front tooth, the hole is usually located on the tongue side.
Third Step: Removing diseased tissue. Using special instruments, your dentist will remove the diseased tissue in the pulp and root canals, essentially stopping the infection and any tooth pain you've been experiencing. The empty pulp chamber and canals are often then disinfected with a special antibacterial solution.
Fourth Step: Protecting the tooth. After some shaping, the pulp chamber and root canals are filled with a special filling to prevent further infection. The access hole is then filled and sealed to complete the procedure. At some point in the future, the tooth typically will need a crown to add support and further protection.
You may have some minor discomfort afterward, but this can usually be managed with a mild pain reliever like ibuprofen or acetaminophen. After a week or so, you'll be good as new—and so will your tooth.

If you’re undergoing your first root canal treatment, it’s understandable if you’re apprehensive. So, let’s cut to the chase about your biggest fear: a root canal treatment doesn’t cause pain, it relieves it — and saves your tooth too.
You need this procedure because decay has spread deep into your tooth’s inner pulp. The infection has already attacked the nerves bundled within the pulp chamber, the source of the pain that led you to us in the first place.
The real concern, though, is the infection continuing to travel through the canals of the tooth root. If that happens, you’re in danger of not only losing the tooth, but also losing surrounding bone, adjacent teeth or damaging other important structures close by. Our goal is simple: remove the infected pulp tissue and seal the empty chamber and root canals from further infection with a special filling.
We begin by numbing the tooth with local anesthesia — you won’t feel anything but slight pressure as we work. After placing a dental dam — a thin sheet of rubber or vinyl — around the affected tooth to maintain a clean work area, we drill a small hole through the biting surface of a back tooth or in the rear surface of a front tooth. We’ll use this hole to access the pulp, where we’ll first remove all the dead and diseased tissue from the chamber. We’ll then disinfect the chamber and root canals with antiseptic and antibacterial solutions.
After some shaping, we’ll fill the chamber and canals, usually with gutta-percha that’s malleable when heated and can be compressed into and against the walls of the root canals to completely seal them. We’ll then seal the access hole.
You may have a few days of mild discomfort afterward, which can be managed generally with pain relievers like aspirin or ibuprofen. Later, we’ll permanently restore the tooth using filling to seal the root canal inside the tooth followed by a custom crown that’s fit over and bonded to the tooth. This will further minimize chances of a re-infection.
If we’ve recommended a root canal, then we think your tooth should be saved instead of extracted. The procedure will end the pain you’ve been suffering and give your tooth a new lease on life.

Primary (baby) teeth might not last long, but their impact can last a lifetime. Their first set of teeth not only allows young children to eat solid foods, but also guide permanent teeth to form and erupt in the proper position.
Unfortunately, primary teeth aren't immune to tooth decay. If the decay is extensive, the tooth may not last as long as it should. Its absence will increase the chances the permanent teeth won't come in correctly, which could create a poor bite (malocclusion) that's costly to correct.
If a primary tooth is already missing, we can try to prevent a malocclusion by installing a “space appliance.” This keeps nearby teeth from drifting into the empty space intended for the permanent tooth. The best approach, though, is to try to save a primary tooth from premature loss.
We can often do this in much the same way as we would with a permanent tooth — by removing decayed material and filling the prepared space. We can also perform preventive applications like topical fluoride or sealants that strengthen or protect the tooth.
It becomes more complicated, though, if the pulp, the interior of the tooth, becomes decayed. The preferred treatment for this in a permanent adult tooth is a root canal treatment. But with a primary tooth we must also consider the permanent tooth forming below it in the jaw and its proximity to the primary tooth. We need to adapt our treatment for the least likely damage to the permanent tooth.
For example, it may be best to remove as much decayed structure as possible without entering the pulp and then apply an antibacterial agent to the area, a procedure known as an indirect pulp treatment. We might also remove only parts of the pulp, if we determine the rest of the pulp tissue appears healthy. We would then dress the wound and seal the tooth from further infection.
Whatever procedure we use will depend on the extent of decay. As we said before, our number one concern is the permanent tooth beneath the primary. By focusing on the health of both we can help make sure the permanent one comes in the right way.

It’s absolutely critical to stop tooth decay and repair any damage to tooth structure. Hopefully, we’ve caught it early in the enamel and dentin where we’re able to repair any holes or “cavities” that might have developed by filling them.
But what if the decay has crept deeper into the interior of the tooth? In this case, you’re at a much higher risk of eventually losing the tooth. If the decay has spread into the bone, a root canal treatment is usually your best option.
The first sign that decay has invaded the pulp, the innermost layer of the tooth, and the root canals may be a severe toothache. It’s different from the wince of pain or discomfort caused by sensitivity to temperature or pressure. Inner decay pain is constant and often excruciating. This is because the infection is attacking the nerves bundled within the pulp chamber.
The pain can last for several days, and then suddenly stop altogether. No pain is good news, right? Wrong — the pain has stopped because the infected nerves have finally died and can no longer signal the brain. The infection, though, is very much alive and will continue to advance toward the root where the damage may eventually cause you to lose the tooth.
A root canal treatment removes decay in the pulp chamber and canals. We first drill an access hole to enter the pulp chamber. Once inside, we use special instruments to completely remove all tissue and disinfect the empty chamber. We then fill the chamber and root canals with a special filling and seal the access hole to prevent further decay. A few weeks later we install a custom crown to protect the tooth further.
If you have a toothache, or you’ve had one that suddenly went away, you should schedule an appointment for a dental examination as soon as possible: this is the only way to accurately determine the cause of the pain. If a root canal is deemed necessary, the sooner we perform it, the less chance the infection will cause you to lose your tooth.
